The Patient Charts – Maternity, dated from 1947 to 1979, came from the Queen Alexandra Maternity Hospital. They are charts and daily reports that include the mother’s name, age, address, next of kin, date of admission, medical officer, details of the delivery and sex of the baby. The charts are on microfiche. These records are held by the Tasmanian Archives. Tasmanian Archives also holds an index to assist with identification of relevant charts.

D75 means these records are closed to the general public for 75 years from the date of the last entry in them. People wishing to access these records should contact the Department of Health and Human Services. Applicants will be required to fill out a Personal Information Protection (PIP) form. Staff at the Tasmanian Archive and Heritage Office may be able to assist you in finding other relevant records.
