The Advice of Discharge Forms from St Catherine's Orphanage are carbon copies of forms completed for the Commonwealth Department of Social Services advising that a child has been discharged from the care of the home. The forms include: name of child; date of birth; date of discharge; and discharge details.

The Advice of Discharge Forms, St Catherine's Orphanage records were held by Sisters of Mercy Archives (North Sydney) until March 2012, when they were transferred to CatholicCare Sydney.

Access Conditions

People seeking access to information will need to verify their identity with a signed statement of request and two forms of identification, one of which should be signed.

There are additional identification requirements for relatives of Care Leavers and Former Child Migrants and those wishing to access the records of someone who is deceased or unable to request the records for themselves.

Where adoption is involved, there are separate requirements related to accessing Social and Medical information in keeping with the Adoption Act 2000.
