Children's Admissions - Application Forms contains the records of admissions of individual children into St Joseph's Orphanage between 1945 and 1979. The titles and dates of the three main forms are Joseph's Orphanage Application for Admission from 1948 to 1956, Aikenhead House Admission from 1945 to 1967, and Catholic (Family) Welfare Bureau Confidential Notification to Aikenhead House from 1968 to 1976.

Records

The form entitled Joseph's Orphanage Application for Admission contains the following fields:

  • the child's name.
  • date and place of birth.
  • baptism details.
  • immunisation details.
  • names of parents, their dates of death if this is relevant, religions, and addresses if still living.
  • address of relatives or guardians.
  • address of clergyman.

Occasionally there is an additional note giving the admission date. There is also a declaration giving the Sisters of Charity full charge of the child.

The form entitled Aikenhead House Admission contains:

  • surname of family.
  • names, religions, addresses, and occupations of parents.
  • child's name, date of birth, sacraments, immunisation record, and schooling.
  • dates of admission.
  • dates of discharge and to whom.

Information about relatives may also be included.

The form entitled Catholic (Family) Welfare Bureau Confidential Notification to Aikenhead House contains:

  • admission and discharge dates.
  • the child's name, date and place of birth, religion, baptism details, school (including place, grade and when last attended).
  • names of parents, whether they are living or deceased, their address, occupations, religions.
  • names, sex, ages, and addresses of siblings.
  • health history, including size of family, types and dates of inoculations, illnesses, current treatments, name of family doctor, sensitivity to drugs, and a signed consent for the administration of anaesthetic.
  • reasons for admission.
  • the circumstances of the family.
  • signature of the person completing the form.
